There are four main functions of the modern anesthesia machine:

  1. Oxygenation - Delivering life-sustaining oxygen with precision and consistency, ensuring the patient's safety and comfort throughout their medical procedure.
  2. The accurate mixture of anesthetic vapors - Achieving the perfect balance of anesthetic agents to maintain patient sedation levels effectively and safely.
  3. Appropriate ventilation - Providing controlled breathing support tailored to each patient's needs, enhancing their experience and outcome.
  4. Reduce exposure of anesthetic vapors to personnel - Minimizing occupational exposure risks to ensure a safer operating environment for healthcare professionals.[2]

The anesthesia machine can be divided into three basic areas:
(1) a high-pressure system,
(2) an intermediate pressure system, and
(3) a low-pressure system.
The high and intermediate pressure systems measure pressure in psi or kPa, while the low-pressure system uses cmH20. The high-pressure system, with its oxygen (2200 psi), air, and nitrous oxide (745 psi) from an E-cylinder, kicks in when the pipeline supply is unavailable. A pressure regulator adjusts this to 45 psi. This system is crucial for remote locations. Meanwhile, the intermediate system, drawing gases from the hospital's 50 psi pipeline, feeds into the flowmeters. The low-pressure system, downstream, regulates fresh gas flow to vaporizers, ensuring a steady supply of volatile anesthetics for patient use.

Gas Flow Through the Anesthesia Machine

The following describes the generalized flow of gas through the modern circle system anesthesia machine:

  1. Gas, whether oxygen, air, or nitrous oxide, enters the machine via the pipeline or cylinder supply. Primarily relying on the pipeline at 45 to 60 psi, the system shifts to the E-cylinder when necessary, using a pressure regulator to stabilize the pressure.
  2. In the intermediate pressure system, gas advances to the flowmeter, where the anesthesiologist carefully adjusts the fresh gas flow rates. In the low-pressure system, pressures drop below 1 psi, measured in cm H20.
  3. Gas may then pass through a variable bypass or measured flow vaporizer, receiving its volatile anesthetic as needed.
  4. Next, gas travels through a unidirectional inspiratory valve for patient inhalation via the circle system's inspiratory limb.
  5. Expired gas traverses the expiratory limb, passing the expiratory unidirectional valve.
  6. When the ventilator is off, or on manual/spontaneous mode, excess pressure is released via the APL valve. This gas is captured by the scavenger system for safety.
  7. Remaining gas circulates through a CO2 absorber, effectively removing carbon dioxide from expired gas, ensuring patient comfort and machine efficiency.
